Dear Valued Patients:
We want you to know that your health and safety is our priority. We are continuing to see patients during normal business hours as usual in our office. In light of the COVID 19 pandemic and in an effort to protect the health and safety of our patients we also offer telemedicine services to our patients. Our providers are currently able to continue to treat you using telemedicine services instead of face-to-face visits in the office for the duration of the pandemic at the discretion of your Primary Care Physician.
The use of telemedicine requires both the patient and the provider to be able to see and speak to each other utilizing a smartphone (either android or iPhone), iPad or laptop with a camera and a microphone.
In order to have a telemedicine visit with your provider , our providers will be using Zoom.
Our staff will schedule your televisit at a specific time just like in-office visits. If you are scheduled for a telemedicine visit please simply go to your email and click the Zoom link ,in the body of the message 5-10 minutes before your scheduled televisit.

Referral Request
If you are requesting a referral, we ask that you notify our office 5 days in advance. That allows our staff adequate timeto prepare your referral. All referral requests for any new medical condition, require an appointment with your physician to determine medical necessity.

1. Appointments
To accommodate all our patients' needs, we offer appointments days, weeks or months in advance as well as same day scheduling. If you have an urgent need please call us 623-738-0440 and we will get you in as soon as possible.
2. Late/ No Show Policy
If you are running late please call our office and we will accommodate you . We do ask that you call at least 24 hours in advance if you cannot make your appointment.

If you have an emergency, please call 911 or go directly to the nearest emergency room. For less urgent medical concerns please call our answering service at and the on call Provider will respond. Routine calls, such as medication refills or referrals will be handled during regular office hours.
5. Referrals/ Pre-Certifications
If you need to see a specialist, your insurance company may require a referral. It is your responsibility as the patient to determine if your insurance requires a referral, to verify that the specialist is on your plan and to obtain a referral from our office before visiting the specialist. New referrals require an office visit for documentation of medical necessity. Referral requests require one to two weeks’ notice before your visit with the specialist.
